  • BLS, PALS, ACLS - AHA or ARC only, required for onboarding
  • License/Certificate Verification -Primary Source within 30 days of start
  • COVID-19 Vaccine - Proof or exemptions must be submitted for approval
  • MMR, Varicella, Hepatitis B, TB Test, Tdap - Immunization or titer proof required
  • Health Statement - signed and dated within 1 year of start
