    • Job Requirements
    • Primary Source Verification of professional license, Nursys, and/or certification, as required for the position ran within 30 days of start updated upon expiration.
    • Proof of vaccine and 1 booster or exemptions must be submitted for approval.
    • Will be completed onsite with CHS on start date
    • Must be within 30 days of start. Negative TB skin test, QuantiFERON Gold or T-Spot. Must state negative results. Updated annually. If positive, must show proof of positive history, chest x-ray within last 5 years and annual TB Questionnaire thereafter.
    • Statement signed and dated within 1 year of start stating traveler is able to work without restrictions. Updated annually.
    • Required from October - March (unless otherwise indicated by CDPH) Medical/Religious exemptions will be sent for approval with a physician's statement(medical) or written statement from traveler(religious).
    • Clinical Only: Skill specific required to be complete within 1 year of start.
