• Certification Details
• AHA or ARC Only BLS Certification required.
• AHA or ARC Only ACLS Certification required.
• Primary Source Verification of professional license ran within 30 days of start updated upon expiration.
• Skill specific required to be complete within 1 year of start (Clinical Only).
• Annual requirement for CHS NIH Certification.
• Proof of vaccine and booster for COVID-19 or exemptions.
• Proof of MMR immunizations or positive titer, or acceptance of declination.
• Proof of Hepatitis B immunizations and positive antibody titer, or acceptance of declination.
• TB Test within 30 days of start and updated Annually.
• Health Statement without restrictions dated within 1 year of start.
• Varicella immunizations proof or acceptance of declination.
• Tdap vaccination within past 10 years or acceptance of declination.
